It is built for people making difficult care decisions: adult children, spouses, and caregivers who need plain-English safety context before a placement, transfer, or tour.

Each facility report can include CMS star ratings, nurse staffing hours, health-inspection deficiencies, immediate-jeopardy and serious harm flags, federal fines and payment denials, and ownership or chain affiliations when those fields appear in public data.

All figures come from public government / CMS sources such as Care Compare, Payroll-Based Journal staffing, penalty files, and ownership disclosures. We compile and explain the record; we do not replace a tour, a clinician, or CMS.gov.
